Residents must have MSP enrollment and a referral to an MSP enrolled medical specialist for services outside their community.

Your travel must not be covered by third party insurance.

a completed TAP form (available from a physician, nurse practitioner or speciality clinic) and
a confirmation number (subsequently received from TAP).
Forms are valid for three months; ensure all details are current.
